Camp Registration
To register for camp, print the following three forms:
Application Form
Parent Release and Insurance Form - please send in with the application.
Medical Form This is a two-part form; one general form to be completed by all campers, and the self-medication form to be completed by campers who will be bringing medications with them. (hand these forms in at camp registration in August). The medical form must be current within two years. A school or camp physical form containing all of the information included on the Pioneer form may be substituted for the Pioneer medical form.
Fill out the application form completely and return it with all appropriate signatures and a $150.00 non-refundable deposit made out to: Pioneer Soccer School. Send the check and the signed registration form to:

Have your doctor complete the medical form and submit it at check-in on the first day of camp.
